For the health and safety of everyone who accesses our facility, we require you to register online and digitally accept our waiver forms.
This includes but it not limited to:
Umpires pre-authorized and/or assigned to officiate games at the Academy
Contractors, Sub-Contractors & Vendors pre-authorized to perform work at the Academy
ALL employees who will be on-site at the Academy must complete their own registration
Guests with scheduled or pre-authorized appointments at the Academy
Please download and complete a W-9 Form if the Academy will be responsible for payment of fees for services performed. You can upload a completed W-9 Form during the registration process.
